Retatrutide in Canada: The Complete 2026 Guide to Status, Trials and Research Supply

Key Takeaways

  • •Retatrutide is not authorized in Canada or anywhere else. It has no Drug Identification Number (DIN), no Notice of Compliance, and it was not on Health Canada's list of new drug submissions under review as of 30 September 2026.
  • •It is one molecule acting at three receptors. Retatrutide (LY3437943) is a lipidated 39-residue peptide that activates the GIP, GLP-1 and glucagon receptors, which is what separates it from semaglutide (one receptor) and tirzepatide (two).
  • •Phase 3 is largely complete. Five large trials have reported, three of them already in peer-reviewed journals, and Eli Lilly has said it plans to file in the United States in the first quarter of 2027.
  • •The headline number depends on the estimand. TRIUMPH-1 was announced at 28.3% mean body-weight reduction and published at 25.0%. Both are correct, they answer different questions, and most coverage never says which one it is quoting.
  • •Canada is a major research site. Ten registered retatrutide studies list Canadian sites, more than 115 site entries across seven provinces, and the lead author of the first retatrutide Phase 3 paper to be published is based in Brampton, Ontario.
  • •Counterfeits are documented, not hypothetical. Australia's regulator tested a product sold as retatrutide in September 2026 and found semaglutide and no retatrutide at all. Identity testing by mass spectrometry is the only thing that separates the two.

Retatrutide is the most talked-about investigational peptide of 2026, and most of what is written about it in Canada is either a year out of date or quietly selling something. This guide pulls together where the compound actually stands: what it is, what every published trial measured, what Health Canada has and has not done, which Canadian sites took part in the research, and how to tell a real retatrutide sample from a vial that only carries the name.

This page is written for laboratory and in-vitro research procurement. It reports what clinical trials recorded in their participants because that is the public evidence base, but it contains no dosing, administration or human-use guidance, and none should be inferred. Every figure below is dated and sourced, and the list of sources is at the bottom.

What Retatrutide Actually Is

Retatrutide was designed at Eli Lilly and first described in detail in Cell Metabolism in 2022, under the development code LY3437943. It is a synthetic peptide built on a GIP-like backbone and engineered so that a single chain can switch on three different hormone receptors.

PropertyValue
Development codeLY3437943
CAS number2381089-83-2
Molecular formulaC221H342N46O68
Average molecular weightabout 4,731 g/mol
Length39 amino acid residues
Non-standard residues2-aminoisobutyric acid (Aib) and alpha-methyl-leucine
LipidationC20 fatty diacid on a lysine side chain, through a gamma-glutamate and AEEA linker
C-terminusAmidated
Elimination half-life (Phase 1b)about 6 days

Two design choices explain most of that table.

The unusual residues resist breakdown. Native GIP and GLP-1 are cut apart by the enzyme DPP-4 within minutes. Aib and alpha-methyl amino acids sit where those enzymes would normally cleave, so the chain survives far longer.

The fatty acid makes it stick to albumin. The C20 diacid binds reversibly to serum albumin, the most abundant protein in blood, which slows clearance dramatically. The same strategy is used in semaglutide and tirzepatide. In the Phase 1b study the half-life came out at roughly six days, which is why every trial since has used a once-weekly schedule.

How a Triple Agonist Works

Retatrutide is an agonist at three receptors, all members of the class B G protein-coupled receptor family. Each one does something different.

  • GLP-1 receptor. Found in the pancreas, gut and brain. Activation increases glucose-dependent insulin secretion, slows gastric emptying and acts on appetite circuits in the hypothalamus and brainstem.
  • GIP receptor. Found in the pancreas and adipose tissue, among others. Activation also supports glucose-dependent insulin secretion and is studied for its effects on fat tissue and lipid handling.
  • Glucagon receptor. Found mainly in the liver. Activation raises hepatic glucose output and fat oxidation, and in animal studies it increases energy expenditure.
One peptide chain, three receptors. Retatrutide is a single molecule shaped to engage the GLP-1, GIP and glucagon receptors, each of which has a different binding pocket.
One peptide chain, three receptors. Retatrutide is a single molecule shaped to engage the GLP-1, GIP and glucagon receptors, each of which has a different binding pocket.

The obvious question is why anyone would add glucagon, a hormone that raises blood glucose, to a molecule studied for metabolic disease. The answer from the preclinical work is that GLP-1 and GIP activity counterbalance the glucose effect, while the glucagon component adds a lever the other two do not have. In the Coskun 2022 mouse studies, body-weight reduction was augmented when glucagon-receptor-driven increases in energy expenditure were added to the reduced food intake driven by GIP and GLP-1 receptor activity.

The potency balance is deliberately uneven. In laboratory receptor assays retatrutide shows more activity at the GIP receptor and balanced, lower activity at the glucagon and GLP-1 receptors. That ratio is the core of the design, and it is the reason retatrutide is not simply "tirzepatide plus glucagon".

Retatrutide is the first of these to complete a Phase 3 program with all three receptors in one molecule. Several other triple agonists are in earlier development, but none has published Phase 3 data.

The Clinical Record, Trial by Trial

Retatrutide's evidence base grew quickly. The timeline below lists every major readout and where its full results were published.

All of these trials were funded by Eli Lilly, and each compared retatrutide against placebo. There is no published head-to-head trial against tirzepatide or semaglutide yet. One against semaglutide in type 2 diabetes (TRANSCEND-T2D-2) is underway with twelve Canadian sites.

The Phase 2 Trials

The 2023 Phase 2 obesity trial is the study that made retatrutide famous. It enrolled 338 adults and ran for 48 weeks.

Three other Phase 2 results shaped what came next:

  • Type 2 diabetes (Lancet 2023). 281 participants across 42 US centres. At 24 weeks, HbA1c fell by up to 2.02 percentage points in the highest arm, against essentially no change on placebo and 1.41 points on the active comparator dulaglutide. Body weight fell by up to 16.9% at 36 weeks.
  • Liver fat (Nature Medicine 2024). 98 participants from the obesity trial with at least 10% liver fat by MRI. At 24 weeks, relative liver fat fell by 81.4% and 82.4% in the two highest arms, and 86% of the highest arm reached normal liver fat (below 5%), against 0% on placebo.
  • Phase 1b (Lancet 2022). The trial that established the roughly six-day half-life, with exposure that scaled proportionally across the arms.

The Phase 3 Trials

Lilly's Phase 3 program is split into two families. TRIUMPH studies obesity and its complications, and TRANSCEND studies type 2 diabetes. The TRIUMPH trials used a basket design, meaning sleep apnea and knee osteoarthritis sub-studies were nested inside the larger weight-management trials and analysed separately.

TRIUMPH-1Obesity or overweight, no diabetes2,33980 weeks-25.0% vs -3.9%Published, NEJM
TRIUMPH-2Obesity with type 2 diabetes1,15280 weeks-18.8% vs -5.1%Published, Lancet

TRANSCEND-T2D-1Type 2 diabetes, diet and exercise only53740 weeks-15.3% vs -2.6%Published, Lancet

Published figures use the treatment-regimen estimand. Topline figures are as released by Lilly and use the efficacy estimand. See the next section for why that matters.

Beyond body weight, the trials measured other endpoints that are worth knowing about:

  • Glucose control. In TRANSCEND-T2D-1, HbA1c fell by 1.69 to 1.94 percentage points across the three arms against 0.81 on placebo, from a baseline of 7.9%. No severe hypoglycaemia was reported.
  • Sleep apnea. In the TRIUMPH-1 sleep apnea sub-study (243 participants with a baseline apnea-hypopnea index of 58.6 events per hour), the index fell by 34.3 events per hour in the 9 mg arm against 9.9 on placebo at 80 weeks.
  • Knee osteoarthritis. In the 574 TRIUMPH-1 participants with knee osteoarthritis, WOMAC pain scores improved significantly more than on placebo. TRIUMPH-4 met its knee pain endpoints too, though commentators noted a large placebo response.
  • Cardiovascular events. TRIUMPH-3 reported a pre-specified analysis of major adverse cardiovascular events with a hazard ratio of 0.82, which was not statistically significant (95% CI 0.55 to 1.22). The dedicated outcomes trial, TRIUMPH-Outcomes, has enrolled about 10,000 participants and has not reported.

Why the Headline Number Keeps Changing

If you have read three articles about retatrutide you have probably seen three different numbers for the same trial. Here is why.

When Lilly announced TRIUMPH-1 in May 2026, the headline was 28.3% mean body-weight reduction in the highest arm. When the full paper appeared in NEJM four months later, the primary result was 25.0%. TRIUMPH-2 went from 20.8% in the press release to 18.8% in The Lancet.

Nobody revised anything. The two figures come from two different estimands, which are formal definitions of the question a trial is answering.

Efficacy estimandTreatment-regimen estimand
Question it answersWhat happens if everyone stays on the assigned treatment? What happens to everyone who was randomized, whatever they did next?
Who stopped treatment earlyHandled as if they had continuedCounted as they actually were
Typical useCompany topline releasesPrimary result in most journal papers
TRIUMPH-1, highest arm-28.3%-25.0%
TRIUMPH-2, highest arm-20.8%-18.8%

The efficacy estimand is always the larger number because it sets aside the participants who stopped, and dropout rates rise with the arm's intensity. Neither figure is wrong, but they cannot be compared with each other, and comparing a retatrutide efficacy figure against a tirzepatide treatment-regimen figure is a common way articles manufacture a gap that is not there. We wrote a full explainer on this: how to read peptide trial results and their estimands.

What the Trials Recorded on Safety and Tolerability

These are observations recorded in trial participants receiving a manufactured drug product under medical supervision. They are reported because they are part of the public evidence on the compound, not as a guide to anything.

  • Gastrointestinal events were the most common. In TRIUMPH-2, diarrhoea was recorded in 27% to 34% of the retatrutide arms against 13% on placebo, and nausea in 14% to 28% against 8%. In Phase 2 these events were partly reduced by starting at a lower level.
  • Dysesthesia is the new signal. Abnormal skin sensations, often described as burning or tingling, were reported in roughly 12% of the two highest TRIUMPH-1 arms against under 1% on placebo, and in 4% to 7% of TRIUMPH-2 arms. This is not a recognised effect of GLP-1 or GIP agonists, and its cause is not established.
  • Heart rate rose. Phase 2 recorded increases in heart rate that scaled with the arm, peaked at 24 weeks, and declined afterwards.
  • Low blood pressure was more frequent. Hypotension was recorded in up to 6% of the highest TRIUMPH-2 arm against under 1% on placebo.
  • Discontinuation rose across the arms. Stopping treatment because of adverse events reached about 11% in the highest TRIUMPH-1 arm against about 5% on placebo.

No regulator has yet reviewed the full safety file. That review is what a marketing authorization decides, and it has not started in Canada.

This is the part most Canadian articles get vague about, so here it is plainly.

Retatrutide is a drug under the Food and Drugs Act, and it is not authorized. In Canada a prescription drug can be lawfully sold only after Health Canada issues a Notice of Compliance and a Drug Identification Number. Retatrutide has neither. Authorized drugs carry an eight-digit DIN on the label, and Health Canada's Drug Product Database shows none for retatrutide.

Health Canada has named it specifically. Public advisory RA-81874, published on 9 April 2026, lists retatrutide among the unauthorized injectable peptide drugs Health Canada has seized, alongside BPC-157, TB-500, GHK-Cu and others. The advisory says the department is working with the Canada Border Services Agency to stop unauthorized shipments entering the country.

A research label does not change the legal status. The same advisory states that products marketed "For Research Use Only, Not for Human Consumption" are not made legal or exempt by that labelling. Material supplied for laboratory research is not a medicine, is not dispensed against a prescription, and is not authorized for administration to people. That is the only basis on which research-grade retatrutide is supplied, including by us.

Enforcement is real. In July 2026 Health Canada announced a permanent court injunction preventing a Quebec company, Canlab Research, from selling injectable peptides. On 13 August 2026 Eli Lilly Canada issued a public safety notice stating that retatrutide has not been approved by Health Canada or any regulator in the world and cannot lawfully be sold to consumers, and asking payment processors, platforms and carriers to stop supporting such sales. Lilly filed six lawsuits against US sellers the same week.

For the wider picture of how Canada regulates peptides, see our guide to whether peptides are legal in Canada.

When Could Retatrutide Reach Canada?

Nobody can give a date, and anybody who does is guessing. Here is what is actually known.

  • No Canadian submission is on the public record. Health Canada publishes a list of new drug submissions under review. Retatrutide was not on it as of 30 September 2026.
  • The first filing is planned for the United States. In July 2026 Lilly said it intends to submit a Biologics License Application to the FDA in the first quarter of 2027, later than an earlier end-of-2026 target, while it completes its manufacturing data package.
  • Lilly does file in Canada. The same Health Canada list shows Lilly's oral GLP-1 agonist orforglipron under review since January 2026. A Canadian retatrutide filing is likely at some point, but its timing has not been announced.
  • Review takes time once it starts. Health Canada's performance standard for a new drug submission is 300 calendar days of review, before pricing and provincial formulary decisions.

Put together, the earliest realistic window for a Canadian decision is after a Canadian submission that does not yet exist. Treat any specific Canadian launch date as speculation.

Retatrutide Trials With Canadian Sites

Canada has been one of the larger contributors to the retatrutide program. ClinicalTrials.gov lists 33 studies of the compound, and ten of them have Canadian sites.

The sites run from Victoria to St. John's, with the heaviest concentration in Ontario and Quebec. There is a Canadian name on the published record too: the TRANSCEND-T2D-1 paper in The Lancet is led by Dr Harpreet Bajaj of LMC Diabetes and Endocrinology in Brampton, Ontario.

Research-Grade Retatrutide Is Not the Trial Drug

These are two different things, and the difference matters for anyone reading the trial literature next to a supplier's listing.

Clinical trial drug productResearch-grade material
What it isA finished, sterile pharmaceutical manufactured under GMPLyophilised peptide powder for in-vitro and laboratory work
Regulated asAn investigational drug under a clinical trial authorizationA laboratory reagent, not a drug
FormulationProprietary, with excipients and a delivery deviceThe peptide alone, typically as a salt
Who it is forEnrolled trial participants under medical supervisionQualified researchers
Trial results apply to it?YesNo

Nothing in the TRIUMPH or TRANSCEND results describes research-grade material. The trials tell you what the molecule does as a manufactured drug product in a controlled protocol. They tell you nothing about the identity, purity or content of a vial bought from a supplier. That has to be established separately, by analysis.

The Counterfeit Problem, in Evidence

Retatrutide's profile has made it one of the most counterfeited names in the peptide market. This is documented by regulators, not just suspected.

  • A "retatrutide" product containing no retatrutide. On 14 September 2026 Australia's Therapeutic Goods Administration published results of testing a product sold as retatrutide after a serious adverse event report. It contained undeclared semaglutide and no retatrutide.
  • A liver injury cluster. On 19 June 2026 Victoria's Chief Health Officer reported six cases of acute liver injury since January 2026 in people who had used unapproved products labelled retatrutide, sold under names including "Reta", "R-10" and "R-20", and said a contaminant was a possible cause.
  • Litigation in the United States. Lilly's August 2026 lawsuits argued that sellers labelling products research use only were in fact marketing them for human use, and that none of the products had been through testing or regulatory review.

For a researcher, the practical lesson is narrow and important. A label saying retatrutide proves nothing. A substituted GLP-1 agonist looks identical as a white powder, dissolves the same way and can even behave plausibly in a crude assay. Only identity testing tells them apart.

Two vials that look identical can hold different molecules. A white powder cannot be identified by eye; a mass spectrum shows which molecule is actually in the vial.

How to Verify a Retatrutide Sample

Retatrutide's size is an advantage here. At about 4,731 Da with a distinctive lipid side chain, its mass is very different from semaglutide (about 4,114 Da) or tirzepatide (about 4,813 Da), so a competent mass spectrometry method separates them cleanly. What to ask for:

  • Identity by mass spectrometry, not HPLC alone. An HPLC purity figure tells you how much of the sample is one main component. It does not tell you what that component is. Look for LC-MS or LC-MS/MS identity confirmation against a reference standard.
  • A purity figure with its method. Chromatographic purity by HPLC, with the method stated. Be aware that it bounds peptide-related impurities only and is not a content measurement.
  • Net peptide content. How many milligrams of actual peptide are in the vial. Lyophilised peptides carry counter-ions and water, so the gross powder weight overstates the peptide.
  • A lot number that matches the vial. A certificate for some other batch tells you about some other batch.
  • A verification path at the laboratory. A report you can look up on the testing laboratory's own website is worth more than a PDF, because a PDF is easy to edit.

Our own certificate for retatrutide is public. SideChain Analytics report COA-2026-SC-02082, dated 16 September 2026, covers lot VPRT20100 of the 20mg vial. It reports identity confirmed against a reference standard by HPLC-MS/MS (MRM transition 947.9 to 136.1), chromatographic purity of 99.7%, and 29.26 mg of net peptide against the 20 mg label claim. It covers that fill only. The 5mg, 10mg and 30mg vials are released to the same greater than 99% HPLC specification, and a certificate for another batch is available on request. Every certificate we publish is on our certificates of analysis page with a link to verify it at the laboratory.

If you would rather not take any supplier's word for it, including ours, our directory of peptide testing labs lists independent laboratories that will analyse a sample you send them. For the analytical detail, see what actually proves a peptide's identity and our step-by-step guide to verifying a peptide in Canada.

Storage and Handling in the Laboratory

Retatrutide is supplied as a lyophilised (freeze-dried) powder, which is the most stable form for a peptide.

  • Store the powder frozen, dry and dark. -20°C is the standard laboratory condition for long-term storage of lyophilised peptide.
  • Keep moisture out. Let a sealed vial reach room temperature before opening it, so condensation does not form on the powder.
  • Avoid repeated freeze-thaw cycles once material is in solution. Aliquoting is the usual practice.
  • Shipping at ambient temperature is normal. Lyophilised peptide tolerates short periods at room temperature, which is how it travels.

Each product page carries the handling notes for that vial.

Frequently Asked Questions

Is retatrutide approved in Canada?

No. Retatrutide has no Notice of Compliance and no Drug Identification Number from Health Canada, and it is not approved by any regulator in the world. As of 30 September 2026 it was not on Health Canada's list of new drug submissions under review.

When will retatrutide be available in Canada?

There is no date. Eli Lilly plans to file in the United States in the first quarter of 2027, and no Canadian submission has been made public. Once a submission is filed, Health Canada's review standard is 300 days, before pricing and coverage decisions.

Is it legal to buy retatrutide in Canada?

Retatrutide cannot be lawfully sold in Canada as a drug for human use. Health Canada named it in an April 2026 advisory on unauthorized injectable peptides and has said that a research-use-only label does not make such products legal for that purpose. Research-grade material is supplied only as a laboratory reagent for in-vitro research.

What is the difference between retatrutide and tirzepatide?

Tirzepatide activates two receptors, GIP and GLP-1. Retatrutide activates those two plus the glucagon receptor, with its activity weighted toward GIP. No head-to-head trial comparing them has been published, so claims that one outperforms the other rest on cross-trial comparisons, which are unreliable.

What did the Phase 3 trials find?

In the published TRIUMPH-1 trial, the highest arm recorded a mean body-weight change of -25.0% at 80 weeks against -3.9% on placebo. Phase 3 trials also recorded improvements in HbA1c, sleep apnea severity and knee osteoarthritis pain compared with placebo. These results describe a manufactured drug product given under protocol.

Why do articles quote different results for the same trial?

Because they mix estimands. Lilly's press releases use the efficacy estimand, which assumes participants stayed on treatment, while journal papers usually lead with the treatment-regimen estimand, which counts everyone randomized. TRIUMPH-1 is 28.3% on the first and 25.0% on the second.

Is research-grade retatrutide the same as the trial drug?

No. The trial drug is a sterile, formulated pharmaceutical manufactured under GMP and given under a clinical trial authorization. Research-grade material is lyophilised peptide for laboratory use, and the clinical results do not describe it. Its identity and purity have to be shown by analysis.

How can I tell if a retatrutide sample is genuine?

Ask for mass spectrometry identity confirmation against a reference standard, an HPLC purity figure with its method, a net peptide content figure, a lot number matching the vial, and a verification link at the testing laboratory. Australian regulators have documented a product sold as retatrutide that contained only semaglutide.
