Leuprolide

Leuprolide (leuprorelin) is a synthetic GnRH (gonadotropin-releasing hormone) agonist nonapeptide (MW ~1209.4 g/mol) that is one of the most widely prescribed hormonal therapies in oncology and reproductive medicine.
Mechanism: Leuprolide is a GnRH agonist approximately 15-100 times more potent than native GnRH. It operates through a "flare then suppression" mechanism: initial administration causes a transient stimulation ("flare") of pituitary gonadotropin release (LH and FSH surge), leading to a temporary increase in testosterone or estrogen over the first 1-2 weeks. With continuous administration, the sustained agonism causes GnRH receptor downregulation and desensitization of pituitary gonadotrophs, resulting in profound suppression of LH and FSH secretion. Testosterone levels in men fall to castrate levels (<50 ng/dL) within 2-4 weeks, and estrogen levels in women decrease to postmenopausal levels. The D-leucine substitution at position 6 of native GnRH confers resistance to enzymatic degradation and increased receptor binding affinity. Depot formulations use biodegradable poly(lactic-co-glycolic acid) (PLGA) microspheres or in-situ gel (Eligard) for sustained release over 1-6 months.
Goserelin

Like other GnRH agonists, goserelin operates through a "flare then suppression" mechanism, achieving castrate levels of sex steroids with continued administration. The implant formulation is available in 1-month (3.6 mg) and 3-month (10.8 mg) presentations.
Mechanism: Goserelin is a GnRH agonist with a D-Ser(tBu) substitution at position 6 and an azaglycine amide at position 10, providing enhanced potency and enzymatic resistance compared to native GnRH. The mechanism follows the "flare then suppression" paradigm: initial administration causes transient stimulation of pituitary gonadotrophs, increasing LH, FSH, and downstream sex steroids (testosterone in men, estradiol in women) for approximately 7-10 days. Continuous GnRH receptor stimulation leads to receptor downregulation and desensitization, suppressing gonadotropin secretion and reducing testosterone to castrate levels (<50 ng/dL) within 2-4 weeks in men, and estradiol to postmenopausal levels in women. The Zoladex implant consists of goserelin dispersed in a D,L-lactic and glycolic acid copolymer matrix that degrades in vivo over 28 days (3.6 mg) or 12 weeks (10.8 mg), providing sustained drug release. Serum goserelin levels peak at approximately 2-3 days post-implant.
Nafarelin

Nafarelin is a synthetic GnRH agonist decapeptide analog (MW ~1322.5 g/mol) with a D-Nal(2) substitution at position 6, administered as a nasal spray (Synarel). Nafarelin is approximately 200 times more potent than native GnRH and is unique among GnRH agonists in its intranasal route of administration, which avoids injections entirely. Like other GnRH agonists, it achieves sex steroid suppression through the "flare then suppression" mechanism.
Mechanism: Nafarelin is a GnRH agonist with a D-2-naphthylalanine (D-Nal(2)) substitution at position 6, providing approximately 200-fold greater potency than native GnRH and resistance to enzymatic degradation. Following intranasal administration, nafarelin is absorbed through the nasal mucosa with a bioavailability of approximately 2-5%. The "flare then suppression" mechanism applies: initial administration stimulates pituitary gonadotrophs, transiently increasing gonadotropin and sex steroid levels. Continuous twice-daily dosing causes GnRH receptor downregulation and desensitization, suppressing LH and FSH secretion and reducing estradiol to postmenopausal levels within 4 weeks. Serum nafarelin peaks at approximately 10-45 minutes after nasal administration. The elimination half-life is approximately 3 hours. The drug is metabolized by peptidases, and the primary route of elimination is via the kidneys.
Buserelin

Buserelin is a synthetic GnRH agonist nonapeptide analog (MW ~1299.5 g/mol) with a D-Ser(tBu) substitution at position 6 and an NHEt modification at position 10. It is approved in the European Union, Canada, Australia, and other countries for the treatment of advanced prostate cancer, endometriosis, and as part of IVF protocols, but it is not approved by the FDA in the United States.
Mechanism: Buserelin is a GnRH agonist approximately 20-40 times more potent than native GnRH. Like other GnRH agonists, it operates through the "flare then suppression" mechanism. Initial administration causes transient stimulation of pituitary gonadotrophs, resulting in an LH and FSH surge with temporary increases in testosterone (men) or estradiol (women) during the first 1-2 weeks. Continuous administration causes GnRH receptor downregulation and desensitization, leading to suppression of LH and FSH secretion. Testosterone falls to castrate levels within 2-4 weeks in men; estradiol falls to postmenopausal levels in women. The half-life of the free peptide is approximately 1-2 hours. It is metabolized by tissue peptidases.
