These purchases occur openly on social media feeds with before-and-after images, injection guides, and testimonials promoting weight loss, muscle recovery, and anti-aging, often paired with discount codes and disclaimers stating this is not medical advice.

Gray Market Operates in Plain Sight

Social media peptide sales appear within wellness content, community forums, and influencer channels, forming a gray market parallel to clinical care. Survey respondents who use peptides inform their doctors about their usage, yet they acquire products lacking pharmaceutical oversight, quality control, purity assurance, or proper dosing guarantees.

The global peptide market surpasses $50 billion in annual sales and expects to nearly double over the next decade. This growth coincides with rising incidents of serious injuries, particularly among those sourcing from unregulated vendors.

Peptide users face these risks despite the products' natural occurrence in the body as short chains of amino acids that regulate hormones, reduce inflammation, and repair tissue. Synthetic versions replicate or boost these natural proteins, with the P in GLP-1 denoting peptide and insulin qualifying as a peptide.

GLP-1 Success Fuels Peptide Expansion

The surge in GLP-1 drugs has popularized injectable wellness treatments. Evan Miller, CEO of Gameday Men’s Health, stated to CNN in April, “The GLP-1s put it on the map.” He added, “And then people were like, ‘Well, what’s next?'”

Dr. Mike Mrozinski, a general practitioner, explained to the BBC that “The success of regulated GLP-1 drugs has ‘normalised’ using a needle, lowering the psychological barrier to self-injection.” This shift prompted interest in other peptides such as BPC-157, TB-500, and Retatrutide.

None of these peptides hold FDA approval for human use. Sellers market them online with labels stating “for research purposes only.”

Imports and Unregulated Sales Tactics

Chinese manufacturers supply peptides to U.S. consumers in large volumes, using the research label as a legal cover. U.S. customs data records $328 million in imports of hormone and peptide compounds from China during the first three quarters of 2025, doubling from $164 million in the same period the previous year.

Ezra Marcus, an investigative journalist who covered the gray market for New York Magazine and appeared on NPR’s Here & Now, noted, “When you go through the process of buying them on the websites, they’ll all say, ‘This is for research purposes only.'” He described it as “a sort of fig leaf they’re using to be able to sell these things that are obviously not for research purposes. They’re for people to inject.”

Marcus obtained a six-month supply of Retatrutide, a GLP-1 developed by Eli Lilly but lacking FDA approval, from a Chinese factory. He connected via a Discord server linked from Reddit, paid $150 in Bitcoin, and received the package two weeks later without a prescription, medical history review, or follow-up care.

The Sunlight survey, released this month, underscores these dose inaccuracies and contamination dangers as key factors in the documented emergency room visits.
